These terms cover text messages (SMS) sent by SAUL ULLMAN M.D. P.A (Ullman Eye Consultants), 5528 North Davis Highway, Pensacola, FL 32503. By providing your mobile number and opting in to receive SMS messages from SAUL ULLMAN M.D. P.A, you agree to receive customer care messages and account notification messages, and you agree to these terms.

About Our Text Messages
Ullman Eye Consultants sends text messages to our patients about their care and their account with our practice. These include:
- Appointment reminders and confirmations
- Scheduling and rescheduling messages
- Follow-up messages after your visit
- Reminders to complete forms
- Billing and payment notices
- Replies to messages and voicemails you send us
We do not send marketing or promotional text messages.
How You Opt In
You opt in by giving us your mobile phone number and signing the text message consent on our patient intake form. Agreeing to receive text messages is not a condition of receiving care. If you do not opt in, we will contact you by phone, mail, or the patient portal instead.
Message Frequency
Message frequency may vary. On average, 1-2 messages per month, mostly around your appointments, plus replies to any messages or voicemails you send us.
Message and Data Rates
Message and data rates may apply. Ullman Eye Consultants does not charge for text messages, but your mobile carrier's standard messaging and data rates apply.
How to Opt Out
Reply STOP to any of our text messages to stop receiving them. Replying UNSUBSCRIBE, CANCEL, END, or QUIT also works. We will send one message confirming that you have been unsubscribed, and you will receive no further text messages from us. You can also call (850) 208-1900 and ask us to stop texting you.
To start receiving text messages again, reply START from your mobile phone. For your protection, carrier rules require that you do this yourself; our staff cannot restart texts for you.

Reply HELP to any of our text messages for help, or call (850) 208-1900, Monday through Friday, 8 a.m. to 4 p.m.
Your Health Information
Text messages are not encrypted. To protect your privacy, our text messages do not include details of your diagnosis or treatment. Please do not text us medical questions or personal health details. Call our office or use the patient portal instead.
Do not use text messages for urgent eye problems. If you have sudden vision loss, eye pain, or an eye injury, call our office right away. In an emergency, call 911.
Privacy
No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties. Your Mobile Number
By opting in, you confirm that you are the subscriber or authorized user of the mobile number you give us. If you change or give up that number, please let us know so we do not text its new owner.
Delivery
Carriers are not liable for delayed or undelivered messages. Delivery of text messages is not guaranteed.
Changes to These Terms
We may update these terms from time to time. Updates will be posted on this page with a new effective date.
